TOPIC: TRANSITION TOOL KIT: The Transition Toolkit is intended to ease the stress of transition for students with disabilities and their families. Our goal is to provide a functional tool that is versatile, portable and useful. For kids with disabilities, the toolkit is a good way to document goals and dreams and map out a way to achieve them. For parents, the toolkit includes tips, techniques and tactics to assure your son/daughter’s successful transition.
